Enroll in a Kaiser Permanente Medicare health plan for Individuals using a government website
Medicare beneficiaries may also enroll in our Medicare health plans for Individuals through the Centers for Medicare & Medicaid Services Online Enrollment Center at www.medicare.gov†. Note: This option is not available for Group Coverage through an employer, trust fund, or union.

You have two ways to enroll in a Kaiser Permanente Medicare health plan for Group Members:
Online: Most people with coverage through their employer group, trust fund, or union can enroll online now.
By mail: Complete the enrollment form in the kit we sent you, and drop it in the mail. Or, contact your organization's benefits administrator to request an enrollment form.

Washington residents: Call 1-800-581-8252, if you live in the following counties: Grays Harbor (98541, 98557, 98559, 98568), Island, King, Kitsap, Lewis (excluding 98591, 98593, 98596), Mason (98524, 98528, 98546, 98548, 98555, 98584, 98588, and 98592), Pierce, San Juan, Skagit, Snohomish, Spokane, Thurston, and Whatcom. You must reside in the Kaiser Permanente Medicare health plan service area in which you enroll.
In California, Maryland and the District of Columbia, Kaiser Permanente is an HMO plan and a Cost plan with a Medicare contract. In Hawaii, Oregon, Washington, Colorado, and Georgia, Kaiser Permanente is an HMO plan with a Medicare contract. In Virginia, Kaiser Permanente is a Cost plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal.
